Medical History Statement webpage.
Each Member who has not replied to a Letter of Termination for Delinquent Dues
and paid all their delinquent dues within the 30 day grace period after the date it was mailed,
who subsequently is permitted to be reinstated through exception by the District and National offices,
is required to complete
a Medical History Statement
and submit it to our Life Insurance carrier.
ALL of his dependents are also required to complete a Medical History Statement of their own.
Life Insurance and AD&D benefits
ARE NOT IN EFFECT until these forms are completed,
submitted and approved by Standard Insurance Company, per the terms of our contract.
Dues are not reduced should the insurance carrier disqualify you and/or a member
of your family,
since the dues amounts are set by license category.
